Seeking a highly experienced, caring case manager or care navigator for a **temporary, private-pay position** supporting a disabled adult in Brattleboro. The work would likely be fairly intensive for the first 1–2 months and then gradually taper as appropriate long-term services are successfully established.
This individual is dealing with a very difficult combination of physical disability, chronic pain, mental health concerns, and challenges managing the practical demands of daily life. I am looking for someone who can bring both strong professional competence and genuine sensitivity to the situation — someone who is patient, respectful, nonjudgmental, and able to develop a trusting relationship while also helping things move forward.
This is not a companion, personal-care attendant, life-coaching, or entry-level case-management position. I am specifically looking for someone with substantial professional experience working with adults who have complex physical and mental health needs and difficulty navigating healthcare and social-service systems.
The individual needs hands-on assistance with:
* Accessing and becoming established with mental health services
* Making calls and completing necessary applications and paperwork
* Coordinating medical, mental health, and community services
* Advocating with medical providers and pharmacies regarding medications
* Organizing appointments and following through with treatment recommendations
* Addressing financial-management difficulties and helping establish appropriate ongoing financial supports
* Developing practical systems that will allow him to function more independently over time
The successful person must be willing to go to the client's home and actively complete tasks alongside him. Simply providing referrals, phone numbers, reminders, or suggestions will not be sufficient. He needs someone who can recognize when he is overwhelmed or struggling, work collaboratively with him, and help him take the next concrete step without being judgmental or taking away his dignity and autonomy.
I am particularly interested in applicants with backgrounds in community mental health case management, intensive case management, medical or behavioral-health social work, disability services, care coordination, HCRS or another Vermont designated mental-health agency, or hospital/community health case management.
LICSW, LMSW, RN, or similar professional credentials are welcome but not necessarily required. **Extensive, directly relevant case-management experience may be more important than a particular degree.**
The position would initially involve approximately 3–4 hours per week, likely in one or two visits, with additional phone/email coordination as needed. The expectation is that the time commitment would decrease to biweekly and eventually monthly as appropriate long-term supports are put into place.
**Compensation: $60–$90/hour depending on experience and qualifications. A higher rate may be considered for an exceptionally qualified candidate. Mileage/travel time can be discussed.**
