A full-time professional role supporting adults with developmental disabilities in Orange County. You'll coordinate services, develop individualized plans, and serve as a key advocate for clients working toward greater independence. What you'll do
Review and analyze assessment data to develop comprehensive Individual Service Plans (ISPs)
Coordinate and attend interdisciplinary team meetings
Prepare Semi-Annual Progress Reports and maintain accurate case records
Train and support direct care staff on goal implementation and documentation
Provide job coaching support and field coverage with clients as needed
Ensure all documentation meets confidentiality guidelines and regulatory standards
Monitor client progress and adjust services to meet evolving needs
Serve as a liaison between clients, families, and service providers
Qualifications
Bachelor's degree required; Human Services field preferred
Two years of experience writing and implementing vocational plans in a program setting preferred
Experience working with individuals with developmental disabilities
Experience designing/implementing behavior modification intervention services a plus
CPR and First Aid certification (must be completed within 30 days of hire)
Clean criminal background check as outlined in DDS licensing requirements (LIC 508)
Reliable vehicle with valid California driver's license and current auto insurance
Strong knowledge of behavior modification and human development
Excellent organizational skills and ability to work independently
Outstanding written and verbal communication skills in English (Spanish a plus)
Demonstrated ability to work effectively on an interdisciplinary team
