Position Summary
Life House Legacy LLC is seeking a dependable, patient, and respectful Direct Support Professional to provide person-centered Supported Living Services (SLS) to an older adult with an intellectual disability. The DSP will promote independence, health, safety, dignity, choice, and meaningful community participation while following the Individual Program Plan (IPP), SLS service plan, physician instructions, and company policies.
Essential Responsibilities
· Provide respectful one-to-one support in the individual's home and community while honoring privacy, rights, preferences, and independence.
· Give reminders and hands-on assistance, as authorized, with bathing, grooming, denture care, dressing, toileting/incontinence routines, and changing clothes.
· Prepare or assist with nutritious meals and snacks; follow physician-ordered dietary guidance, including diabetes-related and sugar-free requirements.
· Assist with medication routines only as trained and authorized, including reminders, documentation, and observation; promptly report errors, refusals, side effects, or health changes.
· Assist with authorized blood-glucose monitoring and recording in accordance with training, written instructions, and company policy; do not perform nursing tasks outside the DSP's training or legal scope.
· Support safe mobility with a cane or walker, fall prevention, short walks, transfers as needed, and safe navigation in the home and community.
· Provide transportation and support for medical/dental appointments, errands, church, recreation, and other approved community activities.
· Assist with household routines such as laundry, dishes, cleaning, trash removal, shopping, and basic meal preparation while encouraging maximum participation.
· Use positive, non-confrontational support when frustration occurs; protect property and safety and follow the approved behavior/emergency response plan.
· Complete accurate, timely service notes, medication/health logs, mileage records, incident documentation, and shift handoffs; maintain confidentiality and HIPAA compliance.
· Communicate professionally with the individual, family/authorized representatives, SLS leadership, IHSS staff, service coordinator, and healthcare team as permitted.
· Recognize and immediately report suspected abuse, neglect, exploitation, significant health changes, and special incidents according to mandated-reporting and NLACRC/Life House Legacy requirements.
Minimum Qualifications
· At least 18 years old with a high school diploma or GED; prior DSP, caregiver, SLS, senior-care, or developmental-disability experience strongly preferred.
· Ability to communicate clearly in English, follow written plans, document services accurately, and use a smartphone or electronic documentation system.
· Patient, reliable, observant, and comfortable supporting an older adult with hearing and vision impairments, diabetes, chronic health conditions, and occasional incontinence.
· Valid California driver's license, reliable insured vehicle, acceptable driving record, and willingness to transport the individual when assigned.
· Ability to pass required DOJ/FBI background clearance, employment verification, health screening/TB clearance, and any other applicable vendor or Regional Center requirements.
· Current First Aid and CPR certification, or ability to obtain before independent assignment; willingness to complete all required orientation and annual training.
· Ability to stand, walk, bend, reach, assist with mobility, and perform household tasks safely, with or without reasonable accommodation.
Preferred Training or Experience
· Supported Living Services, person-centered planning, medication assistance, diabetes awareness, fall prevention, dementia/aging support, positive behavior support, and emergency response.
· Experience working collaboratively with family caregivers, IHSS providers, Regional Center teams, and community healthcare providers.
Schedule Expectations
Candidates should have dependable availability for assigned shifts. Day, evening, overnight, weekend, holiday, on-call, and relief shifts may be available based on the individual's authorized service schedule. Employees may work only scheduled and properly authorized hours. Overtime or schedule changes require advance approval in accordance with company policy.
