Position Summary
Unbreakable Commitments Home & Health is seeking dependable, compassionate, and flexible Certified Nursing Assistants (CNAs) to provide direct care and support to adults living in our residential homes.
This position is ideal for a CNA who enjoys flexible scheduling and is willing to work at different Unbreakable Commitments Home & Health locations based on resident and staffing needs.
A major purpose of the CNA/Float position is to strengthen our staffing team by helping cover:
* Open shifts
* Employee call-ins
* Vacations and requested time off
* Staffing shortages
* Additional resident supervision needs
* Increased resident care needs
* Emergency staffing situations
CNAs may also be offered regularly scheduled shifts depending on availability and the needs of the homes.
Primary Responsibilities
The Certified Nursing Assistant will provide safe, respectful, person-centered care while assisting residents with their activities of daily living and maintaining a safe and supportive home environment.
Responsibilities include:
* Assist residents with bathing, showering, grooming, dressing, and personal hygiene.
* Assist with toileting and incontinence care.
* Assist residents with transfers, repositioning, mobility, and ambulation as appropriate.
* Assist residents with eating and hydration when necessary.
* Observe residents for changes in condition, behavior, mobility, skin condition, appetite, elimination, or overall functioning.
* Promptly report changes or concerns to management.
* Measure and document vital signs when assigned.
* Assist with medication-related responsibilities when properly trained and authorized under company policy and applicable regulations.
* Provide reminders, cueing, redirection, and supervision based on residents’ individual needs.
* Support residents with dementia, traumatic brain injury, developmental disabilities, behavioral needs, and other conditions served by the agency.
* Follow each resident’s Individual Service Plan and established care instructions.
* Maintain resident dignity, privacy, independence, and personal choice.
Staffing & Float Responsibilities
Because this position helps support overall staffing coverage, CNAs must be willing to:
* Work at more than one Unbreakable Commitments Home & Health location when needed.
* Accept available shifts based on their submitted availability.
* Assist with filling open shifts caused by call-ins, vacations, or staffing shortages.
* Respond promptly when contacted regarding available emergency or same-day shifts.
* Work days, evenings, nights, weekends, and/or holidays according to availability and staffing needs.
* Arrive on time and remain for the entire accepted shift unless relieved or authorized by management.
* Communicate availability accurately and promptly.
* Notify management immediately if an accepted shift cannot be worked.
Once a CNA accepts a shift, the employee is expected to report as scheduled.
Resident Safety & Supervision
The CNA will:
* Provide appropriate resident supervision based on each resident’s needs.
* Follow fall-prevention and safety protocols.
* Maintain a clean, sanitary, and hazard-free environment.
* Follow infection-prevention and universal precaution practices.
* Follow emergency procedures and evacuation plans.
* Report falls, injuries, medication concerns, behavioral incidents, suspected abuse or neglect, and other significant events immediately.
* Never leave residents without appropriate staff coverage.
* Follow all resident-specific behavioral and safety plans.
Documentation
Accurate documentation is an essential responsibility of this position.
CNAs may be responsible for completing:
* Activities of Daily Living documentation
* Behavior documentation
* Sleep documentation
* 24-hour observation records
* Vital-sign documentation
* Meal/intake records
* Bowel and bladder documentation
* Medication documentation when authorized
* Incident documentation
* Shift notes and other required resident records
Documentation must be completed accurately and within the required timeframe before the end of the assigned shift.
Household & Daily Living Responsibilities
Because our residents live in residential homes, CNAs also participate in maintaining a safe and comfortable home environment.
Responsibilities may include:
* Preparing or serving meals.
* Assisting residents during meals.
* Light housekeeping related to resident care.
* Resident laundry and linens.
* Cleaning and sanitizing resident-care areas.
* Maintaining resident bedrooms and common areas.
* Assisting residents with activities and social engagement.
* Accompanying residents on community outings when assigned.
Professional Expectations
Employees are expected to:
* Maintain reliable attendance.
* Arrive to work on time.
* Communicate respectfully with residents, coworkers, management, families, guardians, and healthcare professionals.
* Maintain resident confidentiality.
* Refrain from sleeping while responsible for resident care unless specifically permitted under an authorized sleep-staff arrangement.
* Avoid excessive personal phone use while responsible for residents.
* Follow company policies and procedures.
* Follow professional boundaries with residents and families.
* Participate in required training and staff meetings.
* Accept coaching and corrective feedback professionally.
* Maintain all required certifications and training.
Qualifications
Applicants should have:
* Current Certified Nursing Assistant certification or eligibility to work as a CNA.
* Experience working with adults with disabilities, dementia, behavioral health needs, traumatic brain injury, or long-term care needs preferred.
* Ability to work independently.
* Strong observation and communication skills.
* Ability to remain calm during behavioral or medical situations.
* Reliable transportation.
* Reliable telephone communication.
* Ability to pass applicable caregiver/background requirements.
* Ability to meet company training requirements.
* CPR/First Aid certification preferred or ability to obtain certification.
* Valid driver’s license preferred for employees providing resident transportation.
Physical Requirements
Employees must be able to safely perform the essential functions of resident care, which may include:
* Standing and walking for extended periods.
* Bending, reaching, and assisting with repositioning.
* Assisting residents with transfers and mobility.
* Pushing wheelchairs or other mobility equipment.
* Performing personal care and household duties.
* Responding quickly during emergency situations.

Scheduling
This position may include PRN, part-time, full-time, and/or float opportunities depending on current staffing needs and employee availability.
Hours are not guaranteed for PRN employees unless specifically stated in a separate written agreement.
Employees may provide management with their availability and may be contacted when additional shifts become available.
Staff who demonstrate strong attendance, flexibility, quality resident care, and willingness to assist with open shifts may receive priority consideration for additional hours and regularly scheduled positions.
