New in box and includes left and right sides. Does not include bulbs and from after-market manufacturer. Paid $119 at Amazon. The set I installed looks brand new and was fairly easy to install with help from Youtube video (See photo). Way better that frosted up old headlights. Bought one set but Amazon sent me 2 and didn't want to have duplicate sent back.
