The “Direct Support Professional” will have the responsibility for individuals residing in community homes and is responsible for the general health and safety of individuals that reside at WRC. They must comply with all required county, state, federal and other applicable regulatory requirements. Ensure the health, safety and well being of individuals supported. Work directly on implementing individual ISP goals. Develops strong relationship with individual, individual families, SC and all other external organizations as well as the community at large.
Essential Job Functions
• Implements and adheres to all agency policy and procedures
• Completes the Medication Administration training
• Ensures all prescribed medication is dispensed and documented per the Homes’ Policies and Procedures in cooperation with the individual
• Assists the individual in completing necessary medical appointments and follow-ups; and in the direct personal care of the individual as needed.
• Assist the individual with all ADL (activities of daily living)
o Including Laundry, cooking, toothbrushing, toilting, etc.
• Protects the health and safety of individuals
• Maintains the safety of the home by recognizing conditions, which are or could be safety or programmatic hazards and communicates those concerns with the Manager.
• Assists with the maintenance and upkeep of the home
• Distributes individual funds, obtains and submits receipts per the Policies and Procedures.
• Communicates and resolves program and individual concerns with management
• Participates in all required staff meetings
• Adheres to emergency and crisis situation in the appropriate procedures
• Complies with instructions given by the Manager
• Contacts the appropriate people, both professionals and family as directed.
• Ensures effective communication with the individuals supported, families and other people involved in the provision of supports for the individual as directed.
• Transports individuals as assigned in a safe and timely manner for the needs of the individual
• Responds to emergencies in an appropriate and efficient manner.
• Ensures all required documentation and individual’s progress in his/her specific program area is completed in an appropriate and timely manner
• Observes the behavior, attitude, and physical condition of individual; reports significant changes to Manager and/or physician if needed.
• Attend meetings pertinent to consumer well being on an as needed basis.
• Receives 24 hours of training, annually, as required
• Completes tasks and assignments as directed.
• Assumes other responsibilities as required.
Professional Standards:
• Ensure adherence to agency, county, state federal, and other regulators.
• Maintaining positive and effective relationships with families, other care providers, state and federal agencies, outside business vendors, and the general community.
• Maintain required documentation and ensure accurate and organized record keeping.
• Serves as a model of appropriate language, dress and behavior.
• At any time, you may need to be moved from one site to another for WRC to provide care for all our individuals. Refusal to move will result in disciplinary action.
• Assumes other responsibilities as required
Hours:
• Full Time: 37.5 hours per week. Schedule encompasses varying days and hours including observation of all shifts
• Part Time: Less than 30 hours per week; minimum 2 shifts per week
Qualifications:
A. GED/High School Diploma or equivalent
B. Experience working with Individuals with Intellectual Disabilities preferred
C. Valid Pennsylvania driver’s license. Use of personal vehicle required.
