Assist clients with activities of daily living, including bathing, grooming, dressing, toileting, and personal hygiene.
Assist with mobility, transfers, positioning, and ambulation as appropriate.
Prepare and serve meals according to the client's care plan and dietary needs.
Provide assistance with light housekeeping and maintaining a clean and safe environment.
Assist clients with medication reminders, when permitted by the client's care plan and applicable regulations.
Provide companionship and emotional support.
Assist with errands and other approved activities as outlined in the client's care plan.
Observe and report changes in the client's physical, emotional, or behavioral condition to the appropriate supervisor.
Follow the client's individualized service plan and all agency policies and procedures.
Maintain accurate and timely documentation of services provided.
Properly use the agency's Electronic Visit Verification (EVV) system to clock in and out and document services as required.
Maintain client confidentiality and respect the client's privacy and personal rights.
Report accidents, incidents, emergencies, or safety concerns immediately to the appropriate supervisor.
Communicate professionally and respectfully with clients, families, nurses, supervisors, and other members of the care team.
Attend required training, staff meetings, and supervisory or nursing visits.
Perform other duties related to client care as assigned.
