Position: Direct Care/Activities Team – Adult Day Program
Reports to: Direct Care Supervisor
Hours: Full-Time & Part-Time available
Salary:
$19.00 to $22.00 an hour

NO EVENINGS, WEEKENDS OR HOLIDAYS!
Work with adult participants and other staff members to facilitate a variety of fun and interesting onsite and offsite activities, and to provide care for participants.
Position Description
Activities and Direct Care:
Lead or assist with daily onsite and offsite activities
Assist participants with activities of daily living (meals, toileting, transfers, personal hygiene)
Possess or ability to obtain CPR and First Aid certification (training provided)
Possess or ability to obtain QMAP within 45 days of hire (training provided by)
Administer medications following proper policy/procedure (QMAP)
Required: Drive vans/small buses to and from activities
Ability to lift at least 50 pounds
Complete required documentation, including written daily notes, in a timely manner (daily)
Report all incidents and complete incident report if required (verify with management)
Miscellaneous:
Complete assigned web-based trainings
Open/close site as needed
Attend monthly staff meetings and periodic training events
Assist with miscellaneous tasks as needed; other duties as assigned
Willing to drive a.m./p.m. for participant transport as needed

Reliability is essential, with minimum callouts and zero tolerance for no-call no-show. Attending monthly staff meetings and monthly trainings is mandatory, 3rd Thursday of each month from 4:30 to 6:30 pm.
