SOCIAL WORKER (LMSW)

GENERAL DESCRIPTION:

He/She acts as a liaison between participant and family, participant advocate, and assist with necessary duties to help participant achieve assistance from social agencies in the community.  This person also serves as a team member in the overall plan of care assuring the psycho/social needs are met for each participant.

RESPONSIBILITIES:

• Directly responsible to the Social Services Director of THE basics AT JAN WERNER.
• Responsible to follow potential participant from intake/assessment appointment and follow through to actual enrollment.
• Responsible for required documentation from intake through any significant occurrences in the program, such as family matters, assistance from any social agency, and participants status in the program.
• Educating staff, family, and participants on participants rights and responsibilities, services provided at day care, advanced directives and any other subject pertaining to social services available in the community.
• Make necessary referrals to Department of Aging & Disability Services for any potentially qualifying participant. Communicate with assigned case worker at Department of Aging and Disability Services regarding needs of participant.
• Communicate with all parties concerned regarding any changes, problems, or needs which effect psycho/social well being of the participant.
• Counseling as ordered by physician and recommended by the Interdisciplinary Team.
• Responsible for annual updates in required participant documentation in program (e.g. Registration information, U.S.D.A. application, Day Care agreement, Advanced Directives, and Social History)
• Any other duty deemed necessary by the Executive Director or Social Services Director.
• Initial and periodic assessments, communication of participant changes to team members, development of plan of care, 
• Meet documentation requirements in the medical record and the timeframe for the completion of each assessment and plan of care.

MINIMUM QUALIFICATIONS:

• Masters Degree in Social work from an accredited School of Social Work.  Minimum of two years experience in Geriatrics preferred.
