Responsibilities
Provide skilled Physical Therapy or Occupational Therapy services in the patient's home.
Assess the patient's functional needs and provide appropriate treatment.
Monitor patient progress and adjust the treatment plan as clinically appropriate.
Complete accurate and timely clinical documentation.
Maintain professional, ethical, and safe standards of care.
Communicate effectively with the patient, caregivers, and care team.
Qualifications
Physical Therapist:
Active New York State Physical Therapy License.
Graduate of an accredited Physical Therapy program.
Occupational Therapist:
Active New York State Occupational Therapy License.
Graduate of an accredited Occupational Therapy program.
All Candidates:
Strong clinical and communication skills.
Reliable transportation for home visits.
Ability to work independently.
Home care or rehabilitation experience preferred.
